Intensive Support Team (Learning Disability)

The Intensive Support Teams (IST) provides specialist assessment and assertive interventions for young people and adults with a learning disability aged 16 years and above, who are at immediate risk of placement breakdown or hospital admission due to having complex health needs including behavioural distress and/or mental health deterioration.

IST works as part of a multidisciplinary team alongside community learning disability services, education and local authorities. We work in collaboration with service users and their family or carers to understand the reasons for the potential breakdown

The service offers assertive intervention and practical support to reduce the risk of family or placement breakdown.

The team works 7 days per week, 8am to 8pm.
