The discharge planning team work for the district nursing service and complex nursing for Central Lancashire. All team members hold the specialist practitioner qualification in district nursing.
Please note: this service is only available to patients discharged from Royal Preston Hospital or Chorley and South Ribble District Hospital.
The team work collaboratively with the multidisciplinary members of the acute trust, social services, district nurses, private providers and commissioners to ensure that patients are discharged in a timely manner. The team ensures that patients are discharged to an appropriate setting with necessary equipment to meet their nursing needs.
The team leader attends the daily capacity meetings to review capacity demands within the acute trust and reviews the workload of the team in relation to capacity demands. The team leader identifies areas for training and development for the district nursing service so that knowledge and skills are developed to facilitate expedient discharge of patients from the acute sector.
The team liaise with Longridge Community Hospital regarding bed capacity and expedite discharge to this facility.
