We are pleased to welcome you to the Trust’s Patient Safety Incident Response Plan, in line with requirements of the new national patient safety strategy, which has been fully endorsed by our Board of Directors. Our plan sets out our patient safety priorities, what and how we will investigate to ensure we fully understand how incidents happen, how we will learn and where we will focus our improvement work.
